Create a Survey Template

Surveys allow you to receive the feedback you need to keep your customers happy. In Survey Templates, you can create, edit, preview, and remove surveys.
Instructions
Navigate to Configuration > Templates > Survey Templates
Select Create Survey Template
Fill in fields
Name: Add a name for the survey; this will only display for internal use
Survey Type: Choose if you wish to create a Facility or Session survey
Message: Add text to display in the email and survey page your customer receives
